Notes
from the School Nurse
Preschool, Kindergarten, 6th Grade and 9th Grade Physical Exams
As in the past, students entering kindergarten and 9th grade are required to turn in a current school physical exam (done within 1 year) at registration in August 2009. If physicals are not turned in
by the 1st day, students are not allowed to attend and will be sent home.
6thGrade Physical Exams
A change in Illinois State law requires all students entering
6thgrade provide a school physical exam at registration days in
August. The physical needs to be current-done at the doctor’s office within 1
year. 6thgraders need to have these on file by the 1st day
of school. If physicals are not
turned in by the 1stday, students are not allowed to attend school
and will be sent home.
** Parents: please do not wait until August to schedule these appointments as the doctor’s offices and health department are extremely busy at that time. Avoid the summer rush!
** A sports physical does not qualify as the Illinois school physical exam.
Parents, please remember to fill out the health history and sign it.
Diabetic testing with a BMI (Body Mass Index) is to be
required to be done during all Illinois physicals. Please discuss this portion
of the history with your doctor.

kindergarten

of the following occurs: (i) the child presents proof of a completed eye
